Arrange a blood test

Please only request a blood test appointment if it has been requested by the surgery. Arranging an appointment can be done via 2 methods

Once you have had your blood test, please return and complete an online medication review if requested to via text. We will only contact you if there are any abnormalities found. You can view your results via online access.

Adults

For Option 1:

• Please call 01926331401 to arrange your blood test appointment if you have received correspondence from us requesting you to complete a blood test

For Option 2:

• Step 1: Obtain a print out of your blood test request by calling reception on 01926 331401
• Step 2: Book your appointment online

Children

(PLEASE NOTE: they do not accept booking via the telephone). Also ensure you have a printed blood form supplied by us.
