When receiving care privately it is important that this remains in line with NHS and NICE guidelines. To that effect we have a process in place for ADHD assessments to ensure robust measures are taken prior to us taking over prescribing. Below outlines the process involved. It is important that you make your specialist team aware of this to ensure we can safely share your care.
A copy of the private provider declaration form is available for download below. This is just for information as your specific form will be supplied to you when your GP makes the referral.
- Patient requests private referral to Consultant Psychiatrist
- Consultant Psychiatrist has deemed patient meets DSM-V criteria and ADHD diagnosis given
- Consultant Psychiatrist has initiated and stabilised patient on medication
- Shared care Agreement (SCA)sent to GP surgery by Private Consultant
- Consultant to sign Private Provider Declaration
Sent via Meets Management Hub OR Text with document attached sent to patient to take to Consultant
- Signed SCA and Private Provider Declaration received
- Shared Care Prescribing code added to patient's records and medication issued on repeat
