Sick (fit) notes
A sick note (officially called a fit note or Med3) is a document issued by a GP or hospital doctor that confirms you’re not fit for work due to illness or injury. It may also include recommendations for a phased return or adjustments at work.
You can self-certify using a form (SC2) for up to 7 days, or using your employer’s self-certification form.
If your employer insists on a fit note for less than 7 days of illness, we may be able to to complete a private sick note. There is a charge for this service, click here to find out more and complete the private request.
Requesting a sick note
Patients can submit a request for a sick/fit note without an appointment. However, please note that a Clinician may contact you to discuss your request.
If your request is approved, we aim to issue the sick notes within 3 working days using your chosen method.
Getting back to work
You should go back to work as soon as you feel able to. You do not need to see a healthcare professional again to go back to work.
Your employer may ask for confirmation that you’re fit for work, but healthcare professionals do not have to provide this. Depending on your job, you may need to meet other requirements before you can return to work after being ill.
