Contraceptive Review “Pill Check” & Medication Review

If you have been advised by the surgery to submit a contraceptive pill review please use this form. You will need an up-to-date weight and blood pressure. We have a blood pressure machine accessible to patients at the surgery, you do not need an appointment for this, please come in at any time.
